Support Coordination (Waiver Funded)

Support coordination involves the locating, coordinating and monitoring of the medical, social, habilitation, education and any other community service needed by the participant and his or her family. The Support Coordinator assists the participant and his or her family in choosing and coordinating with qualified providers for the needed services to ensure consistency of services. The participant may also choose to direct their own services, and assistance in doing so is also offered by the Support Coordinator. Ongoing contact is then provided to the participant and his or her family to ensure that services are carried out as planned. The participant’s progress is monitored and the Support Coordinator continues to work as an advocate for the participant to ensure continued services.
