The "Activity and Sensor Monitoring" program is a new service offered through the Clearfield County Area Agency on Aging (CCAAA) through the PA Dept. of Public Welfare Waiver Services. The program employs sensor based technology on a 24/hr. a day basis by remotely tracking the activities of daily living of individuals to assist them in living safely and securely at home. Clearfield Community Nurses will be working closely with the Clearfield County AAA to monitor collected data received from a patient's home. To be eligible for the service an individual must be 60 or older, nursing home eligible and meet certain financial guidelines. Eligibility is determined through the CCAAA. In the future the service will be available to private pay individuals.

Quiet Care is the system that was chosen by the Clearfield Community Nurses for the Activity and Sensor Monitoring program. The system utilizes 5 or more wireless motion detectors strategically placed in a client's home. The detectors collect and track an ongoing picture of an individual's normal daily routine and environment temperature. The collected data is sent to a secure website where it is monitored 24/hrs. a day, 7 days a week. When abnormal data is received caregivers on the established call plan will be notified. The CCAAA and Clearfield Community Nurses will also be able to monitor the collected data for changes from the established normal behavior pattern of the client. The activities monitored are as follows:

• *meal prep (did the person eat)
• *wake-up (did they get up)
• *suspected falls (stayed in B. R. for an extended time)
• *overnight bathroom use (an increase or decrease)
• *overall activity (lack of)
• *temperature in the client's home

The Quiet Care technology enables the staff of Clearfield Community Nurses to intervene and provide services in a timely manner when "out of parameter" readings are detected and allows individuals to remain in the comfort and security of their own homes. Appropriate intervention at the appropriate time by home care agencies caring for the elderly and chronically ill is imperative. The Quiet Care system provides an accurate picture of an individual's behavior pattern at home to the staff of both the CCAAA and Clearfield Community Nurses at all times. Family members who live close by or far away will also be able to access the information from their computers providing them with peace of mind and less worry overall regarding their loved one.
